How to Use

Assuming you have downloaded and installed Uniform Invoice Software:

  1. Download Veterinary Invoice Template. Double-click the downloaded file to extract all files.
  2. Open the template "veterinary.xlsx" or "veterinary-uis.xlsx" with Microsoft Excel.
  3. Click "Design Mode" on the "Invoice" Excel ribbon tab to put it into design mode.
  4. Enter your business information and replace the default logo image to make it your own. (How?)
  5. Click "Design Mode" again to exit design mode.
  6. Create invoice (tutorial movie)!

Running a service related to veterinary medicine? This bill format is designed for you. The free sample veterinary invoice template offers a blank invoicing form for you to itemize your items and calculates sub-totals, taxes and totals automatically.

As a vet billing template this format does not contains the shipping address section that could be found on many templates here on InvoicingTemplate.com. The section that usually places the shipping address fields is now a big "Pet Info" box where you can write down a brief introduction about the pet (dog, cat, etc.) or the symptom, pet health record, or something like dog vaccination record.

You can change the label "Pet Info" to anything that meet your business requirements. For example "Job Info" etc. This way, the template can be used by many other business types, small businesses, service companies like writers, designers, consultants. The underlying database field name was "Summary", which is a custom field (user-defined field) supported by Uniform Invoice Software. By using this template with Uniform Invoice Software, you have a veterinary invoicing software program that is able to create beautiful and professional bills.

Since an invoice form is used as a receipt or bill, usually given from a seller / service provider to the buyer. This document services as a legal form that can list the products or services included as well as the required payment terms. All these can be detailed with this blank billing form.

On the top of the form, you'll see the section for you to detail your own business information. Including business name, address, contact information, email and web address etc. If you are a licensed veterinarian, it is a good idea to detail this fact too - this improves client communication and showcase your expertise in veterinary medicine.

The main part of the form allows you to itemize the medical service / goods provided.

On the bottom of the form, there is a fillable / editable "Notes" field allows you to fill in comment text for the current bill. This template has the default taxing settings as what implemented in the default template shipped with Uniform Invoice Software app. The tax names are by default set to GST and PST. But you an easily change this by clicking the

"Settings button on the "Invoice" ribbon tab, and then go to the "Taxes" tab.
