This free template is an editable version of the General Vendor Invoice Template published on the California State web site. The original version is an image titled as "Sample Vocational Rehabilitation Vendor Invoice". Our template is Excel spreadsheet. PDF version is downloadable too.
We create this template as a sample of customization, to show you how Uniform Invoice Software can work with different form layouts and designs. This template is a direct successor of our cn003 service invoice template in Excel, which is a generic one in our Service Invoice Templates. We modify it to imitate the layout of the official vendor invoice, and meanwhile make sure the new layout work with Uniform Invoice Software by adhere to the rules detailed on customizing invoice template.
On the vendor information section on top of the form, note a line of "Federal Tax I.D. Number" is added. This could be FEIN or SS#. In the "Bill To" section, we reuse the original "Client#" and titled it with "DOR Number". If you have both customer# and DOR authorization number to be included on your invoices, simply add a custom field for the DOR number to the "Invoice Header" database table. We also add 3 columns to the original Excel invoice template, i.e. "Consumer Name", "Authorization #Number" and "Service Date". Please refer to General Vendor Invoice Template and Invoicing Guidelines for Vendors for more information on usage and purpose of these fields.
The "Signature" line on the bottom of the form is not a custom field. It is there for handwriting signature. This means anything you type into the field will not be saved to the backend Access (or Microsoft SQL Server) database.
The line of "California Sales Tax" is an additional line to the original official design. Making it a separate field enables you to add it to reports as a separate column. The default tax rate is set to 6.5%, as shown on California City & County Sales & Use Tax Rates and Research California Sales Tax Rates. To change you tax rate, click "Settings", and then go to the "Taxes" tab. Visit our tax invoice templates for more samples.
