The form features a cartoon lawyer image on the top left of the sheet.
The image shows as a watermark behind the "Legal Service Provider Name" text label. It is set to 10% opacity in Adobe Photoshop so that the watermark does not obscure the front text.
This template shows one tax at the bottom of the sheet. By assuming all the items are taxable, we removed the "Taxable" checkboxes from this template, and the tax is calculated using a formula like this:

Both the "Tax" and "Total" cells show the "USD" currency symbol.
