InvoicingTemplate.com offers this medical bill template in two editions - one freeware edition, and another edition that should work with Uniform Invoice Software. Uniform Invoice Software, originally named Excel Invoice Manager, is our billing software and invoicing software that has the ability to make a simple Excel spreadsheet format become powerful medical billing software.

This free template is the first of our serial of medical billing forms. Originally provided on UniformSoft.com at Medical Invoicing Layout, this template was designed for those hospitals and health care providers who offer medical care for infants, children and adolescents. The other medical billing templates on this serial will offer more variants and formats for you. While the term "medical invoice" is as bare bones as a name can get, medical invoices can actually be a source of confusion if not designed to provide information in a clear and organized manner. A medical invoice can be used at any health care service and assists the health care service provider to maintain health care records of patients. It is used to bill the patient and contains detailed information on the type of treatment or medical service provided to the patients at their time of hospitalization.

Although this free template, like all the other templates here on InvoicingTemplate.com, was created based on the sales invoice template shipped with Uniform Invoice Software - this is necessary to make sure it works with Uniform Invoice Software, being a medical bill means the form contains lots of new fields were added that could not be found on the default template shipped with Uniform Invoice Software, including: Date of service (date field), additional payment fields that are line itemed next to description - Total Fee (editable within invoice), CoPay (editable within invoice), Insurance Reim (editable within invoice), Ins Contract Adjust (editable within invoice), Patient Responsibility (calculated automatically with the this formula: Total fee - CoPay - Insurance Reim - Ins Contract Adjust). Since the medical invoice format is designed for those who offer medical care for infants, children and adolescents, the "Bill To" and "Ship To" fields found on a general invoicing form are utilized a little differently. The "Bill To" would represent the head of household account member (i.e. Dad), while "Ship to" would be the actual patient information - This allows you to easily deal with families with multiple children.

On the bottom of the invoice form there are a group of checkboxes let you choose the payment type - Check, Visa, MasterCard, Amex, Discover. The signature and the additional "date" field are for handing writing. A medical invoice usually contains name of the medical organization, patient information such as their name, address, contact, name of the physician, and comprehensive details on the treatment performed and also information on medications sold to them. If you are using Uniform Invoice Software, note that the newly added checkboxes, which could not found on the default template shipped with Uniform Invoice Software, will be saved to database when the "Save To DB" button is clicked, and will be restored to the saved data if an invoice is reloaded from the backend database. This is a good sample of using custom fields with Uniform Invoice Software, demonstrating that custom fields can not only work with general text fields, but also controls like checkboxes. If you are interesting in the implementation details of these checkboxes on the template, the secret behind the payment method checkboxes is in fact very simple - each payment type check box is linked to a hidden cell. It is this hidden cell Uniform Invoice Software operates on to affect the payment type options.

If you use the Uniform Invoice Software edition of this template, note that an invoice is marked as "paid" if the "Balance Due" cell is zero. By default, a paid invoice is not editable. Contact us for a hidden option if you need to be able to edit paid invoice. This design also implements a new feature, that if you fill in the "card holder name", the invoice will be paid. For this reason, if you run the Statement report, the "payment total" calculation will not work. We recommend you to remove this part from your statement report. This is because the template allows you to enter payment information on the invoice form; there are no separate payment records.

Since this form is designed in Microsoft Excel spreadsheet format, customizing the layout is easy and simple, as it will be shown in the other medical billing formats in this serials of templates.
