This Excel medical invoice template is suitable for general hospital, pharmacist, clinic and home health agency that is giving out the bill, and could be by a doctor, physician, medical expert or medical center to indicate the amount of expenses against medical services or treatment in details provided by them to a patient.
On the top of the spreadsheet, you'll first notice there is a logo image showing "InvoicingTemplate", which is a place holder of your own logo image. Next to the logo image is your hospital, clinic or home health care business name, slogan, street address, city state and ZIP, phone number, web address and etc. This should all be placed with your own information. On the top-right corner, you'll find the letters "MIDCICAL INVOICE" that are all in upper case, which indicates the type of this business document and usually you don't need to modify it.
The previous example of medical records invoice, Medial Invoice Template (1), was designed for those hospitals and health care providers who offer medical care for infants, children and adolescents. This is true to another Medical Billing Invoice Form here on InvoicingTemplate.com, at Medial Invoice Template (2). With this free Doctors Receipt Template, we want to make the format as general as possible. In the "Patient" section, you'll find fields for Hospital No., Name, Address, Phone, Email, Patient Age, Consultant, Payment Mode, Bed No., Admission Date, and Discharge Date. The details section includes 5 columns - #, PARTICULARS, QTY, RATE and AMOUNT. On the bottom of the invoice form, there is one tax designed by default. Another custom field, Med Claim, is also added. The "Total" in this Medical Payment Receipt Template is then calculated with this Excel formula: subtotal + tax - Med Claim. Another two fields on the bottom of the form, i.e. PAID and TOTAL DUE, appear on most formats available here on InvoicingTemplate.com. If you don't need these cells (or any other cells), simply move them out of the printable section. Whether you are a medical office, pharmaceutical sales, or other health care business, you can use our templates for free. You can use our templates as a starting point, adjust and customize the template to suit your own business.
