Designed for private caregivers and independent healthcare providers, this template is a direct successor to Service Invoice Template (cn003).
The "Job Description" field is a multiple-line text field. In Microsoft Excel, this is archived by pushing down the "Wrap Text" button on the "Alignment" tab of the "Home" ribbon tab. To start a new line when inputting text, you press the "ALT" and "ENTER" keys on your keyboard. You can enter any additional information into the description field, including insurance information.
